DO YOU HAVE DIABETES, HIGH BLOOD PRESSURE, CHOLESTEROL or WEIGHT PROBLEMS?
Join one of our professionally facilitated group learning programmes to:
  • Discuss & understand how modern diabetes treatments & lifestyle habits can improve your health
  • Share experiences with others
  • Create your own personal health plan
Sessions run by Peta Fenn (RGN, MSc, PG Dip Ed)

Programmes can be contracted by organisations, such as Primary Care Trust’s, General Practices and health and sports centres, businesses or patient groups. Programmes are run at a local venue provided by the Contracting organisation. The programmes have been designed to the standard required by The National Institute of Clinical Excellence (NICE) criteria and are audited.

Our structured education programmes aim to provide you with the information you need to improve your weight, blood pressure, cholesterol and/or diabetes control and get you and your partner/spouse feel confident that you are up-to-date on all aspects of today’s recommended care.  You will have the opportunity to share your learning experiences and exchange tips for success with other people in a similar position.

We have two main programmes on offer:
  1. Managing your: Weight /High Blood Pressure/Cholesterol /Impaired Glucose Tolerance (IGT)/ Type 2 Diabetes, health problems
  2. Managing your insulin treatment: for people with Type 1 diabetes and people with Type 2 diabetes on or changing to insulin treatment

Week 1
Cardiovascular Disease, Diabetes and your health - understand more
  • Up-date on the science-base, screening, treatments and technologies for good health.
  • Assessing and understanding your personal health measures and issues
Week 2
Cardiovascular Disease, Diabetes and your lifestyle - everyday issues
  • Food matters
  • Activity matters
Week 3
Cardiovascular Disease, Diabetes your medicines and healthcare
  • Understand your medicines & how to make them work for you
  • Know how the NHS can help you
  • Identifying your health action plan
We are currently working with some GP Practice’s in Somerset who are funding the delivery of these education programmes, so that you do not have to pay.  We hope this will roll out to other Practices and are currently exploring this possibility with other GP Practices and Primary Care Trusts.  Why not speak to your GP or Nurse and see if they would be interested in holding a course in your Practice?

2. Managing your insulin treatment: for people with Type 1 diabetes and people with Type 2 diabetes who have or are changing from diabetes tablets to insulin.

Be your own Doctor, Dietitian and Laboratory Technician - everyday lifestyle issues

Understand more on the strengths & weaknesses of your medicines and your healthcare services

  • Side effects, including hypoglycaemia, weight, driving and travel issues " Know how the NHS can help you
  • Identifying your health action plan Start your Personal Health Record

We also:

Provide personalised, small group educational sessions, i.e. for families or for a group of friends who share the same health goal, designing individual programmes to your requirements.

Give public presentations on these health issues, talks for charities and other voluntary organisations and up-date day or half day sessions.
