We offer a personalised service tailored to your lifestyle and

achieve the best solutions for your life with diabetes. This is by helping you

an achievable Personal Health Plan.
- Understand your health needs.
- Explore
your concerns and provide support during difficult times.
- Manage
your diabetes well during difficult times such as starting on insulin,
admission to hospital, life and workplace changes, and new
challenges.
- Keep
up-to-date with latest developments in care, new technologies and treatments
available with advice on how to access these.

Who is providing the service?
|
Peta Fenn has 20 years plus experience gained
as a Diabetes Specialist Nurse in the NHS. She is also a Lecturer in
cardiovascular disease and diabetes, and has worked at universities in the
South West UK. She has an extensive knowledge and understanding of the daily
challenges and issues for people living with diabetes, high blood pressure,
cholesterol and weight problems.
She is offering a personalised,
convenient service which, aims to support you in being proactive in minimising
your ill health risk from your diabetes and related health
problems. |

The Somerset Nuffield Hospital and St. Albans
Exeter.
Or Your Home
We can also provide personalised, small group
consultations, i.e. for you and your family or for a group of friends who share
the same health goal.

Home visits |
The consultation rate will be charged at the above
rate and additional charges maybe made for travel in excess of a 30 mile road
radius from Taunton. Please email or call us to
discuss your needs
Insured Patients
If you
have private medical insurance please contact your insurance company prior to
your consultation to ascertain if you are eligible for reimbursement.
